Order
The Registrar is directed to annotate the Register to show that, for a period of 18 months, you, James Lunderegan must comply with the following conditions of practice:
1. You must confine your professional practice to work for a single employer.
2. You must not carry out any driving duties as part of your professional practice until you have completed a thorough assessment of your driving by your employer and your employer is satisfied that you are safe to drive.
3. You must place yourself and remain under the indirect supervision of a workplace supervisor registered by the HCPC or other appropriate statutory regulator and supply details of your supervisor to the HCPC within 2 weeks of commencing employment. You must attend upon that supervisor as required and follow their advice and recommendations.
4. You must work with your workplace supervisor to formulate a Personal Development Plan designed to address the following areas of your practice:
• Decision making in pressurised situations
• Redacted
5. Within three months of commencing employment you must forward a copy of your Personal Development Plan to the HCPC.
6. You must meet with your workplace supervisor on a monthly basis to consider your progress towards achieving the aims set out in your Personal Development Plan.
7. You must allow your workplace supervisor to provide information to the HCPC about your progress towards achieving the aims set out in your Personal Development Plan.
8. You must provide a Report to the HCPC from your work place supervisor 2 weeks before the next review, that sets out your progress towards the aims set out in your Personal Development Plan.
9. You must remain under the care of your General Practitioner and inform them that you are subject to these conditions.

11. You must keep your professional commitments under review and limit your professional practice in accordance with the advice of your General Practitioner.
12. You must cease practising immediately if you are advised to do so by your General Practitioner. You must inform the HCPC within seven days of receiving this advice.
13. You must inform the HCPC within seven days if you take up professional work.
14. You must inform the HCPC within seven days if you take up work requiring registration with a professional body outside the United Kingdom.
15. You must inform the HCPC within seven days of returning to practice in the United Kingdom.
16. You must inform the HCPC within seven days of becoming aware of:
A. any patient safety incident you are involved in;
B. any investigation started against you; and
C. any disciplinary proceeding taken against you.
17. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of registration);
C. any prospective employer for professional work (at the time of your application);
D. the workplace supervisor required by these conditions.
Notes
This order will be reviewed by the Committee no later than 18 March 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.
