Order
That the Registrar is directed to annotate the HCPC Register to show that for a period of 18 months from today, you, Miss Sidanisile Ndlela, must comply with the following conditions of practice:
1. You must not undertake musculoskeletal ultrasound scans, except under the direct supervision of a Radiographer or Doctor who is fully qualified in musculoskeletal ultrasound and is registered as such with the appropriate regulator.
2. You must not autonomously report on musculoskeletal ultrasound scan findings.
3. You must arrange for an audit of the following areas of your practice by your workplace supervisor every month:
i. Abdominal ultrasounds
ii. Gynaecological ultrasounds
iii. Doppler Ultrasounds
4. You must arrange a monthly audit supervisory meeting with your workplace supervisor to discuss the audit results.
5. You must send to the HCPC a copy of the audit results within seven days of receipt of such results.
6. You must send to the HCPC a written record of the discussion with your workplace supervisor within seven days of each audit supervisory meeting.
7. You must inform the HCPC within seven days if you cease to be employed by your current employer.
8. You must inform the HCPC within seven days if you take up any other or further professional work.
9. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application);
D. any organisation through which you are undertaking professional training.
