(2) If you undertake any activity for which your HCPC registration is required, you must inform the HCPC as soon as reasonably practicable of the name of your employer (or other organisation) for, or through, which you undertake that activity.

(3) If you undertake any activity for which your HCPC registration is required you must ensure that there is appointed a Clinical Supervisor who is able to make an assessment of your clinical work.

(5) You must as soon as reasonably practicable, inform the HCPC of the identity of the appointed Clinical Supervisor.

(6) You must meet with the appointed Clinical Supervisor not less frequently than monthly and ensure that when you meet with your Clinical Supervisor you discuss a selection of cases you have dealt with as a clinician, the cases to be selected randomly by the Clinical Supervisor. This will include an audit of any documentation completed as part of the cases reviewed.

(7) You must secure from your Clinical Supervisor a report, setting out the Clinical Supervisor’s assessment of the standard of your clinical work and documentation in the proceeding period. The report must be submitted to the HCPC not less than two weeks before the date of each review of this Order.
