Order

ORDER: The Registrar is directed to record the following conditions on the Registrant:

1. You must place yourself and remain under the supervision of a workplace supervisor registered by the HCPC or other appropriate statutory regulator and supply details of your supervisor to the HCPC within 7 days of securing employment, paid or unpaid. You must attend upon that supervisor as required and follow their advice and recommendations.

2. You must work with your supervisor to formulate a Personal Development Plan designed to address the deficiencies in your practice as identified by the South West Ambulance Service NHS Trust to include but not limited to:

i. Providing targeted “Worsening Care Advice” to patients.
ii. Identification and appropriate management of Red flags in patients.
iii. Following Triage processes and standard operating procedures.

3. You must meet with your supervisor monthly to review your performance relating to the deficiencies highlighted above and submit a report from your supervisor to the HCPC commenting on your performance 2 weeks prior to the review date of this order.

4. You must register with and remain under the care of a general practitioner and/or appropriate health professional and follow their guidance.

5. You must keep your professional commitments under review and limit your professional practice in accordance with the advice of your general practitioner or any other health professional that you are registered with.

6. You must provide the HCPC with the contact details of your general practitioner and/or health professional within 7 days of the Operative Date or, should your general practitioner and/or health professional change.

7. You must inform the HCPC within 7 days if you take up any other or further professional work.

8. You must notify any future employers that your registration is subject to these conditions.

9. You must inform the HCPC within 7 days of becoming aware of:

i. any patient safety incident you are involved in;
ii. any investigation started against you; and
iii. any disciplinary proceedings taken against you.

10. You must inform the following parties that your registration is subject to these conditions:

i. any organisation or person employing or contracting with you to undertake professional work;
ii. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
iii. any prospective employer for professional work (at the time of your application).

11. You must allow the HCPC to share, as necessary, details about your performance, compliance with, and/or progress under these conditions with:

i. any organisation or person employing or contracting with you to undertake professional work;
ii. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
iii. any prospective employer for professional work (at the time of your application).

Notes

This order will be reviewed by the Committee no later than 24 March 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.

Right to Apply to the Court

Under Article 31(12) of the Health Professions Order 2001, an application may be made to the appropriate court to terminate the order or revoke or vary the conditions imposed.
